Choose the Preserved Bone that matches the Rare item's class, apply it, then take the item to the Well of Souls. Desecration adds one Unrevealed Desecrated modifier and removes a random modifier first when the item's modifier slots are full. Revealing normally offers three choices. A normal item is limited to one Desecrated modifier and cannot be Desecrated again after gaining one.
